Okay, this based on the new Pokemon Mystery Dungeon game, which ties into the title. You can only be a character from the game, however, since this is a new game, Piplup, Chimchar, Turtwig, and Riolu have been added, so you can be them, too. You have to fill out a form to join.
Species: Torchic (Human)
Team Name: Fire Storm
First in command: Pyre
Second in command: Turtwig
Third in command: Pikachu
Game: Darkness Exploration

If you wanted to have something like this you'd need to put the sign-up in the Sign-Up forum.
Also, it needs a plot, otherwise people could just be running around going "YAYAYAY I'M A POKEMON!!"
Also, it's a good idea to join some RPs first, before you make your own.
Just trying to tell you before some people come and be nasty. Believe me, some people actually will just get the nearest mod they see and won't even tell you what you did wrong.
On a brighter note, good sign-up form. Just you'd need to add essentials like Personality and History (although that isn't really essential, it's some-what optional, but alot of RPs think it's a good idea to have it)

Also, magic kid is right, you always need a plot with your RPs. and that joining RPs before making your own gives you the right idea of how to handle it.
You also need to see that the plot has to go beyond the game. How should be people get started in this RP? Who is involved, where are they, what is going on, when is it happening and why? You need to really map out the situation, or else people are going to get stuck and/or very confused.
As for the sign up, what you have is fine, but you also need to add stuff like description, personality, and history. Otherwise, the characters don't really have any depth to them, making them seem pretty bland and unrealistic.
Continue to work on it, and check other RPs that are active to see how its down.
